Immunizations Needed for School

In order to attend class in Minneapolis Public Schools, students must show that they have had the required immunizations or that they are exempt.

Immunization Checklist  This simple check list makes it easy to see which shots (and how many) are required at each grade level. This flier is also available in Spanish, Hmong, Somali, Russian and Vietnamese.

Be sure you and your family are immunized against measles to protect yourselves. 

 

·         Children should get their first MMR (measles, mumps, rubella) shot at 12 months of age and a second MMR shot before Kindergarten.

·         Children 6-12 months of age who will be traveling internationally to areas where measles is common (particularly Kenya, Africa) or where outbreaks are occurring (including Europe) should check with their healthcare provider about getting an early dose of MMR before traveling.

·         Adults born in 1957 or after should make sure they’ve had one MMR shot. If you were born 1956 or earlier, you probably had measles as a child and don’t need the MMR shot.

 

  

Immunization Waiver - Students can be legally exempt from some or all immunizations for medical reasons or because of conscientiously held beliefs of the parents/guardians. To claim an exemption, complete one of the waivers with appropriate signatures on the backside of the

Pupil Immunization Record  (Spanish)  with the appropriate signatures. Medical exemptions must be signed by a health care provider. Conscientious exemptions must be signed by the parent/guardian and notarized.

 

Legal Requirements for Documenting History of Chickenpox Disease:

As of Sept. 1, 2010, a parent's/guardian's signature is no longer acceptable to document history of a child's varicella (chickenpox) disease in either kindergarten or seventh grade. One of the following is needed to document a student's history of chickenpox disease:

  • the signature of a healthcare provider along with the date of the child's chickenpox illness,
  • the signature of a healthcare provider along with a statement that the parent's or legal guardian's description of the child's chickenpox disease history is indicative of past chickenpox infection, or
  • the signature of a healthcare provider or representative of a public clinic along with laboratory evidence of the child's chickenpox immunity.
